Permission Slip, Health Form, & Troopmaster Info Form
for Troop 33 Activities
The following are links to the permission slip that is used for Troop 33. Each Scout must have this permission slip filled out prior to attending a Troop 33 event. Any new Scout joining Troop 33 must fill out a Health Form as well.
New Scouts need to fill out the Troopmaster Info Form as well so we can know any and all necessary phone numbers so we can reach family members in an emergency.

Parents who would like to go camping with Troop 33 are encouraged to read the attached Parent Guidelines document.
